I just reworked the application to look like this:

Looks pretty good.

I'd add a hint of grey to the page to make the user fields stand out more, and a few minor tweeks to formatting. Not that there was anything wrong with the original, just making it look a little different from the AU form.

Very pleased that you took the time to name all the fields properly too! Little details like that make us look quite competent.

Minor nitpick; if you use number fields for the phone number, you can't enter leading zeros eg (07) becomes (7), 855-0123 becomes 855-123 etc. Also all the numeric fields were 1-31 (including the phone number ones!) I've changed months to be 1-12, years to be 1800-2100, and phone numbers to 'text'.

The new form is in /files/ and I think it's about ready for use.

Probably not noticeable, but there was a tiny text box in the FirstName area that was annoying me. Also centred the signature + date table. Etc. --Master5o1 00:02, 3 December 2009 (UTC)
